Synopsis "Therapies Delivrees Par Les Defibrillateurs Automatiques Implantables (in French)"

La thérapie appropriée efficace est l'objectif essentiel du défibrillateur automatique implantable. la survenue d'une thérapie inappropriée constitue un événement potentiellement grave. Méthodes: Nous avons mené une étude ayant inclus 80 patients implantés de DAI au service de cardiologie de l'hôpital Habib Thameur de Tunis entre janvier 2010 et juin 2019. Résultats: L'âge médian des patients était de 58 ans (52,25-65,75) avec un sex ratio de 12,1H/F. Durant un suivi médian de 48 mois, 695 thérapies ont été recensées: 62% de stimulation anti-tachycardique (ATP) et 38% de chocs avec une prédominance des thérapies appropriées (87,8%).L'incidence des thérapies était de 4,72 thérapie/patient-année à un an, 3,28 thérapie/ patient-année à 3 ans et 0,78 thérapie/ patient-année à 5 ans. En analyse multivariée, les facteurs indépendants prédictifs de décès étaient les thérapies inappropriées (RR = 1,102 avec un IC95 % de (1,026-1,184), p=0,008), la cardiopathie ischémique (RR = 2,965 avec un IC95 % de (0,931-9,444), p=0,066) et la FEVG inférieure à 29% (RR = 2,340 avec un IC95 % de (0,942-5,809), p=0,067).
